Do you ever wonder which code is used to report local anaesthetic during scaling? Or who is considered the responsible provider when your hygienist takes radiographs that are interpreted by an associate? You are not alone, let us help you navigate the ODA Suggested Fee Guide with confidence. This course will reinforce the importance of procedure code selection as it relates to the intent of treatment and record keeping. Attendees will learn how thorough documentation and precise coding are vital to ethical, patient-focused care, ensuring consistency, supporting informed consent, fostering professional responsibility, and maintaining public trust in dentistry. The course will also draw from practical examples found in dental offices and explore how carriers leverage claim analytics to spot emerging patterns and flag procedure codes that may raise concerns.

PM Session: Fundamentals of Dental Recordkeeping
Category 1 – 4 CE Credits

Description:
Professional, legal and ethical responsibilities require that dentists maintain complete records documenting all aspects of the patient’s dental care. The chart “tells the story”, beginning with the patient’s medical and dental history, continuing through diagnostic procedures and diagnosis, consent to treatment and treatment plan, and on to progress notes and referral documents. Charting is, quite simply, part of care. The RCDSO’s Dental Recordkeeping courses offer practical advice for understanding the dental recordkeeping requirements in Ontario and how to incorporate them into daily practice.
Learning Objectives:
By the end of this session, participants will be able to:
- Explain the six components of a patient record that must be included to satisfy professional, legal and ethical expectations for documentation.
- Describe the content that must be included in each of the areas of the patient’s record.
- Identify opportunities to strengthen their own practice and apply the fundamental principles of dental recordkeeping required to demonstrate they are practicing to the standard of care.
